Pattern Overview
Blood Stasis is a chronic pattern in TMJ pain, characterized by disc displacement or degeneration, fibrin deposition in the joint cavity, synovial microcirculatory disturbance, fixed pain, and limited movement. The core pathogenesis is Blood stasis in the TMJ collaterals, causing synovial microcirculatory disturbance, fibrin deposition, and chronic pain.
Key Symptoms and Differentiation Points
| Differentiation Dimension | Manifestations |
|---|---|
| Primary Symptoms | Fixed jaw pain, limited mouth opening, clicking, disc displacement |
| Associated Symptoms | Dark complexion, purple-dark lips, tenderness on palpation |
| Tongue and Pulse | Purple-dark tongue with petechiae, hesitant pulse |
| Pathogenesis | Blood stasis in the TMJ collaterals |
| Etiology | Chronic TMJ pain, disc displacement, trauma, prolonged inflammation |
Acupuncture Point Prescription
| Point | Location | Function |
|---|---|---|
| Xiaguan (ST7) | In the depression on the lower border of the zygomatic arch, below the temporal process of the zygomatic bone | Key point for TMJ pain; invigorates blood in the joint |
| Jiache (ST6) | In the depression on the lower border of the zygomatic arch, about 1 cun anterior to the angle of the mandible | Local point; relaxes the masseter muscle |
| Hegu (LI4) | Dorsum of the hand, between the 1st and 2nd metacarpals | Regulates Qi and invigorates blood |
| Xuehai (SP10) | On the medial thigh, 2 cun above the patella | Invigorates blood and resolves stasis |
| Ashi points | Tender spots around the TMJ area | Local blood invigoration and pain relief |
Clinical Recommendation: Filament needle, reducing method; may combine with cupping on the local area to enhance blood stasis resolution.

Modifications
| Associated Symptoms | Additions/Subtractions |
|---|---|
| Disc displacement | Add Dan Shen, San Leng to break up stasis |
| Fibrin deposition | Add San Leng, E Zhu to break up stasis |
| Chronic pain | Add Ru Xiang, Mo Yao to relieve pain |
| Limited mouth opening | Add Shen Jin Cao, Tou Gu Cao to relax sinews |
Western Mechanism Reference
Disc displacement or degeneration with fibrin deposition in the joint cavity, synovial microcirculatory disturbance, and chronic inflammation. Blood Stasis corresponds to TMJ pain with internal derangement, where fibrin deposition and synovial microcirculatory disturbance contribute to disc displacement and chronic pain.
