Pattern Overview
Phlegm Wheezing is a common pattern in asthma, characterized by airway mucus hypersecretion, wheezing with phlegm sound, copious sticky sputum, and chest tightness. The core pathogenesis is Phlegm-turbidity obstructing the Lung, causing Lung Qi to fail to descend, mucus obstruction, and wheezing.
Key Symptoms and Differentiation Points
| Differentiation Dimension | Manifestations |
|---|---|
| Primary Symptoms | Dyspnea, wheezing with phlegm sound, copious sticky sputum |
| Associated Symptoms | Chest tightness, poor appetite, heavy sensation, difficulty expectorating |
| Tongue and Pulse | Pale and swollen tongue with white greasy coating, slippery pulse |
| Pathogenesis | Phlegm-turbidity obstructing the Lung, Lung Qi failing to descend |
| Etiology | Poor diet, Spleen deficiency generating Phlegm, Phlegm hiding in the Lung |
Acupuncture Point Prescription
| Point | Location | Function |
|---|---|---|
| Fenglong (ST40) | 8 cun above the lateral malleolus, 2 cun lateral to the tibial crest | Transforms Phlegm and Dampness |
| Feishu (BL13) | On the upper back, 1.5 cun lateral to T3 | Back-Shu point of Lung meridian; regulates Lung Qi |
| Zhongwan (RN12) | 4 cun above the umbilicus | Front-Mu point of Stomach; harmonizes the Stomach |
| Pishu (BL20) | Lower back, 1.5 cun lateral to T11 | Back-Shu point of Spleen meridian; strengthens Spleen Qi |
| Dingchuan (EX-B1) | 0.5 cun lateral to GV14 (below C7) | Extra point specifically for asthma; stops wheezing |
Clinical Recommendation: Filiform needle, even method; moxibustion may be added to warm and transform Phlegm.
Herbal Formula
Base Formula: San Zi Yang Qin Tang (三子养亲汤) + Er Chen Tang (二陈汤)
Composition:
Perilla Seed (Su Zi), White Mustard Seed (Bai Jie Zi), Radish Seed (Lai Fu Zi), Tangerine Peel (Chen Pi), Pinellia (Fa Ban Xia), Poria (Fu Ling), Licorice (Gan Cao)
Action: Transforms Phlegm, descends Qi, and stops wheezing.
Modifications
| Associated Symptoms | Additions/Subtractions |
|---|---|
| Copious sticky sputum | Add Gua Lou, Bei Mu to moisten and transform Phlegm |
| Chest tightness | Add Gua Lou, Zhi Ke to relieve chest tightness |
| Poor appetite | Add Sha Ren, Mu Xiang to harmonize the Stomach |
| Chronic wheezing with Qi deficiency | Add Huang Qi, Dang Shen to supplement Qi and stabilize the exterior |
Western Mechanism Reference
Airway mucus hypersecretion with mucus plug obstruction of small airways, airflow limitation, and MUC5AC overexpression. Phlegm Wheezing corresponds to asthma with mucus hypersecretion, where goblet cell hyperplasia and MUC5AC overexpression contribute to mucus plug formation and airflow obstruction.
