Pattern Overview

Wind-Cold-Damp Bi is a chronic pattern following ankle sprain, characterized by cold-induced stiffness, chronic ankle pain, and aggravation in cold weather. The core pathogenesis is invasion of Wind-Cold-Damp pathogens into the ankle meridians following incomplete healing, leading to persistent pain and stiffness.

Key Symptoms and Differentiation Points

Differentiation DimensionManifestations
Primary SymptomsStiffness and pain in the ankle, worse in cold or damp weather
Associated SymptomsRelief with warmth, limited range of motion, heavy sensation
Tongue and PulsePale red tongue with thin white coating, floating and tense pulse
PathogenesisWind-Cold-Damp pathogens invading the ankle meridians
EtiologyIncomplete healing of ankle sprain, exposure to cold and damp

Acupuncture Point Prescription

PointLocationFunction
BL60 KunlunDepression between lateral malleolus and Achilles tendonUnblocks the Bladder meridian; treats ankle pain
KI3 TaixiDepression between medial malleolus and Achilles tendonTonifies Kidney Qi; strengthens the ankle joint
GB40 QiuxuDepression anterior and inferior to the lateral malleolusSource point of Gallbladder meridian; treats ankle and foot pain
Ashi pointsTender spots around the ankleLocal pain relief and meridian unblocking

Clinical Recommendation: Filiform needle, even method; moxibustion may be added to warm the meridians and disperse cold.

Modifications

Associated SymptomsAdditions/Subtractions
Severe cold painAdd Fu Zi, Gui Zhi to warm meridians
Pronounced stiffnessAdd Shen Jin Cao, Tou Gu Cao to relax sinews
Chronic weaknessAdd Huang Qi, Dang Shen to supplement Qi

Western Mechanism Reference

Cold-induced stiffness, reduced local blood flow, and increased muscle tension around the joint. Wind-Cold-Damp Bi corresponds to chronic ankle instability with cold sensitivity, often seen in incompletely rehabilitated sprains.
