Pattern Overview
Wind-Cold-Damp Bi is a common pattern in ankle pain, characterized by cold pain, joint stiffness, and aggravation in cold or damp weather. The core pathogenesis is invasion of Wind-Cold-Damp pathogens into the ankle meridians, causing obstruction of Qi and Blood flow and joint dysfunction.
Key Symptoms and Differentiation Points
| Differentiation Dimension | Manifestations |
|---|---|
| Primary Symptoms | Cold pain and stiffness in the ankle joint, worse in cold/damp weather |
| Associated Symptoms | Relief with warmth, limited range of motion, heavy sensation |
| Tongue and Pulse | Pale red tongue with thin white coating, floating and tense pulse |
| Pathogenesis | Wind-Cold-Damp pathogens obstructing the ankle meridians |
| Etiology | Exposure to cold and damp, inadequate ankle protection |
Acupuncture Point Prescription
| Point | Location | Function |
|---|---|---|
| BL60 Kunlun | Depression between lateral malleolus and Achilles tendon | Unblocks the Bladder meridian; treats ankle pain and swelling |
| KI3 Taixi | Depression between medial malleolus and Achilles tendon | Tonifies Kidney Qi; strengthens the ankle joint |
| GB40 Qiuxu | Depression anterior and inferior to the lateral malleolus | Source point of Gallbladder meridian; treats ankle and foot pain |
| Ashi points | Tender spots around the ankle | Local pain relief and meridian unblocking |
Clinical Recommendation: Filiform needle, even method; moxibustion may be added to warm the meridians and disperse cold.

| Pronounced stiffness | Add Shen Jin Cao, Tou Gu Cao to relax sinews |
| Heavy sensation | Add Fang Ji, Yi Yi Ren to drain Dampness |
Western Mechanism Reference
Joint inflammation and stiffness with cold-induced synovial fluid viscosity increase and local vasoconstriction. Wind-Cold-Damp Bi corresponds to cold-induced joint stiffness and pain, often seen in early-stage osteoarthritis or after cold exposure.
