Pattern Overview
Kidney-Yang Deficiency is a severe pattern in allergic rhinitis, characterized by HPA axis dysfunction, severe nasal congestion, cold intolerance, frequent urination, and generalized immune dysfunction. The core pathogenesis is Kidney-Yang deficiency failing to warm the body, leading to HPA axis dysfunction, reduced cortisol levels, and impaired immune regulation.
Key Symptoms and Differentiation Points
| Differentiation Dimension | Manifestations |
|---|---|
| Primary Symptoms | Severe nasal congestion with clear discharge, cold intolerance, frequent urination |
| Associated Symptoms | Cold extremities, fatigue, pale complexion, lower back soreness |
| Tongue and Pulse | Pale and swollen tongue with thin white coating, deep and weak pulse |
| Pathogenesis | Kidney-Yang deficiency with HPA axis dysfunction and immune dysregulation |
| Etiology | Aging, chronic illness, congenital deficiency, overwork |
Acupuncture Point Prescription
| Point | Location | Function |
|---|---|---|
| Shenshu (BL23) | Lower back, 1.5 cun lateral to L2 | Back-Shu point of Kidney meridian; warms Kidney-Yang |
| Mingmen (GV4) | On the midline, between L2 and L3 spinous processes | Warms Kidney-Yang and strengthens the body |
| Guanyuan (RN4) | 3 cun below the umbilicus | Warms Yang and supplements Qi |
| Feishu (BL13) | On the upper back, 1.5 cun lateral to T3 | Back-Shu point of Lung meridian; supplements Lung-Qi |
| Yingxiang (LI20) | In the nasolabial groove, at the level of the midpoint of the lateral border of the ala nasi | Opens the nasal passages |
Clinical Recommendation: Filament needle, reinforcing method; heavy moxibustion is strongly recommended at Shenshu and Mingmen to warm Kidney-Yang.
Herbal Formula
Base Formula: Jin Gui Shen Qi Wan (金匮肾气丸) + Yu Ping Feng San (玉屏风散)
Composition:
Aconite (Fu Zi), Cinnamon (Rou Gui), Rehmannia (Shu Di), Dioscorea (Shan Yao), Cornus (Shan Zhu Yu), Poria (Fu Ling), Moutan (Mu Dan Pi), Alisma (Ze Xie), Astragalus (Huang Qi), Atractylodes (Bai Zhu), Saposhnikovia (Fang Feng), Licorice (Gan Cao)
Action: Warms Kidney-Yang, tonifies Qi, strengthens the defensive exterior, and regulates immunity.
Modifications
| Associated Symptoms | Additions/Subtractions |
|---|---|
| Severe cold intolerance | Add Ba Ji Tian, Yin Yang Huo to warm Yang |
| Frequent urination | Add Yi Zhi Ren, Sang Piao Xiao to astringe the Kidneys |
| Lower back soreness | Add Du Zhong, Xu Duan to strengthen the lower back |
| Profuse nasal discharge | Add Wu Wei Zi, Wu Mei to astringe and stop discharge |
| Fatigue | Add Huang Qi to enhance Qi supplementation |
Western Mechanism Reference
HPA axis dysfunction with reduced cortisol levels, generalized immune dysfunction, and decreased stress response. Kidney-Yang Deficiency corresponds to allergic rhinitis with HPA axis dysfunction, where reduced cortisol levels and impaired immune regulation contribute to severe allergic symptoms.
