Pattern Overview
Cold-Dampness is a common pattern in lower back pain, characterized by
**cold and heavy pain in the lower back, aggravated on rainy days, and
relieved by warmth. The core pathogenesis is invasion of Cold-Damp
pathogens into the lumbar meridians**, where cold causes contraction and
dampness causes heaviness, obstructing the meridians and impairing
Qi-Blood circulation.
Key Symptoms and Differentiation Points
| Differentiation Dimension | Manifestations |
|---|
| Primary Symptoms |
|---|
aggravated on rainy days |
| Associated Symptoms |
|---|
extremities, heavy sensation |
| Tongue and Pulse |
|---|
moderate pulse |
| Pathogenesis |
|---|
meridian obstruction |
| Etiology |
|---|

Acupuncture Point Prescription
| Point | Location | Function |
|---|
| BL23 Shenshu | Lower back, 1.5 cun lateral to L2 |
|---|
Yang, disperses Cold and eliminates Dampness |
| DU4 Mingmen |
|---|
| Warms Yang and disperses Cold, unblocks collaterals and stops pain |
| BL40 Weizhong |
|---|
| Relaxes sinews and activates collaterals, benefits the lower back and
knees |
| Ashi points | Tender spots around the lower back |
|---|
and unblocking for pain relief |
Clinical Recommendation: Filiform needle,
even-reinforcing-even-reducing method; moxibustion is strongly
recommended to warm the meridians, disperse Cold, and transform
Dampness.
Herbal Formula
Base Formula: Gan Jiang Ling Zhu Tang (甘姜苓术汤)
Composition:
Licorice (Gan Cao), Dried Ginger (Gan Jiang), Poria (Fu Ling),
Atractylodes (Bai Zhu)
Action: Disperses Cold and eliminates Dampness, warms and unblocks
the meridians.
Modifications
| Associated Symptoms | Additions/Subtractions |
|---|
| Severe cold with intense pain |
|---|
disperse Cold |
| Heavy Dampness | Add Cang Zhu, Yi Yi Ren to enhance drying Dampness |
|---|
| Soreness and weakness in lower back and knees |
|---|
to supplement Kidney and strengthen the lower back |
| Lower limb numbness |
|---|
and unblock collaterals |
Western Mechanism Reference
Muscle spasm with cold sensitivity, cold-induced vasoconstriction, local
ischemia, and decreased pain threshold. Cold-Dampness lower back pain
corresponds to cold-induced muscle spasm, myofascial pain syndrome, and
acute attack after lumbar cold exposure, commonly seen during weather
changes, prolonged exposure to damp environments, and inadequate lumbar
warmth protection.
