Pattern Overview

Damp-Heat Accumulation is the most common pattern in acute gout,

characterized by **urate crystal deposition, acute joint inflammation,

redness, swelling, burning pain, and fever**. The core pathogenesis is

accumulation of Damp-Heat in the joints, causing acute inflammatory

response and severe pain.

Herbal Formula

Base Formula: Si Miao San (四妙散) + Bai Hu Jia Gui Zhi Tang

(白虎加桂枝汤)

Composition:

Atractylodes (Cang Zhu), Phellodendron (Huang Bai), Achyranthes (Niu

Xi), Coix Seed (Yi Yi Ren), Gypsum (Shi Gao), Anemarrhena (Zhi Mu),

Cinnamon Twig (Gui Zhi)

Action: Clears Damp-Heat, reduces inflammation, and relieves pain.

Urate crystal deposition activating the NLRP3 inflammasome, IL-1β

release, and acute inflammatory response. Damp-Heat Accumulation

corresponds to acute gouty arthritis, where monosodium urate crystal

deposition triggers intense inflammatory responses, including NLRP3

inflammasome activation and IL-1β release.
