Pattern Overview
Wind-Cold-Damp Bi is a common pattern in carpal tunnel syndrome,
characterized by **nerve compression with cold sensitivity, wrist pain,
and numbness aggravated by cold weather**. The core pathogenesis is
invasion of Wind-Cold-Damp pathogens into the wrist meridians,
causing obstruction of Qi and Blood flow and compression of the median
nerve.
Key Symptoms and Differentiation Points
| Differentiation Dimension | Manifestations |

Acupuncture Point Prescription
| Point | Location | Function |
|---|
| PC7 Daling | Center of the wrist crease |
|---|
Pericardium meridian; treats wrist pain |
| PC6 Neiguan | 2 cun above the wrist crease, between the tendons |
|---|
Luo-connecting point; regulates Qi and Blood in the wrist |
| LI5 Yangxi | On the wrist crease, radial side |
|---|
and stiffness |
| TE5 Waiguan | 2 cun above the wrist crease, dorsal side |
|---|
the Triple Energizer meridian; treats wrist pain |
Clinical Recommendation: Filiform needle, even method; moxibustion
may be added to warm the meridians and disperse cold.
Herbal Formula
Base Formula: Huang Qi Gui Zhi Wu Wu Tang (黄芪桂枝五物汤)
Composition:
Astragalus (Huang Qi), Cinnamon Twig (Gui Zhi), White Peony (Bai Shao),
Ginger (Sheng Jiang), Jujube (Da Zao)
Action: Warms the meridians, supplements Qi and Blood, disperses
cold, and unblocks collaterals.
Modifications
| Associated Symptoms | Additions/Subtractions |
|---|
| Severe cold pain | Add Fu Zi, Xi Xin to warm meridians |
|---|
| Pronounced numbness |
|---|
|
| Weakness | Add Dang Gui, Chuan Xiong to nourish Blood |
|---|
Western Mechanism Reference
Nerve compression with cold sensitivity, local vasoconstriction, and
reduced nerve perfusion. Wind-Cold-Damp Bi corresponds to carpal tunnel
syndrome exacerbated by cold exposure, where reduced blood flow to the
median nerve contributes to symptom aggravation.
