Pattern Overview
Bladder Damp-Heat is the most common pattern in acute urinary tract infections, characterized by acute bacterial cystitis, burning urination, frequent urination, urgency, yellow or turbid urine, and lower abdominal pain. The core pathogenesis is accumulation of Damp-Heat in the Bladder, causing bacterial colonization, mucosal inflammation, and urinary irritation.
Key Symptoms and Differentiation Points
| Differentiation Dimension | Manifestations |
| :— | :— |
| Primary Symptoms | Burning urination, frequent urination, urgency, yellow or turbid urine |
| Associated Symptoms | Lower abdominal pain, feverishness, thirst, irritability |
| Tongue and Pulse | Red tongue with yellow greasy coating, slippery and rapid pulse |
| Pathogenesis | Damp-Heat accumulating in the Bladder |
| Etiology | Poor hygiene, sexual activity, urinary stasis, heat-dampness invasion |
Acupuncture Point Prescription
| Point | Location | Function |
| :— | :— | :— |
| Pangguangshu (BL28) | Lower back, 1.5 cun lateral to S2 | Back-Shu point of Bladder meridian; regulates bladder function |
| Zhongji (RN3) | 4 cun below the umbilicus | Front-Mu point of Bladder meridian; promotes urination |
| Yinlingquan (SP9) | Below the medial condyle of the tibia | Transforms Dampness and drains Heat |
| Qugu (RN2) | 5 cun below the umbilicus | Regulates urination and relieves lower abdominal pain |
Clinical Recommendation: Filament needle, reducing method; may combine with moxibustion on Zhongji to promote urination.
Herbal Formula
Base Formula: Ba Zheng San (八正散)
Composition:
Plantago (Che Qian Zi), Akebia (Mu Tong), Polyporus (Zhu Ling), Alisma (Ze Xie), Rhubarb (Da Huang), Gardenia (Zhi Zi), Licorice (Gan Cao)
Action: Clears Bladder Damp-Heat, promotes diuresis, and relieves urinary irritation.
Modifications
| Associated Symptoms | Additions/Subtractions |
| :— | :— |
| Severe burning sensation | Add Jin Yin Hua, Lian Qiao to clear Heat and detoxify |
| Fever | Add Shi Gao, Zhi Mu to clear Heat and generate fluids |
| Lower abdominal pain | Add Yan Hu Suo, Mu Xiang to regulate Qi and stop pain |
| Hematuria | Add Bai Mao Gen, Xiao Ji to cool blood and stop bleeding |
| Recurrent infections | Add Huang Qi, Dang Shen to supplement Qi and strengthen immunity |
Western Mechanism Reference
Acute bacterial cystitis with inflammatory response, bacterial colonization, and mucosal irritation. Bladder Damp-Heat corresponds to acute bacterial cystitis, where bacterial colonization and mucosal inflammation contribute to burning urination and urinary frequency.
