Pattern Overview
Yin-Yang Deficiency is an end-stage pattern in type 2 diabetes, characterized by severe β-cell failure, multi-organ dysfunction, diabetic nephropathy, neuropathy, cold intolerance, and fatigue. The core pathogenesis is prolonged illness consuming both Yin and Yang, leading to severe metabolic dysfunction, organ damage, and systemic decline.
Key Symptoms and Differentiation Points
| Differentiation Dimension | Manifestations |
| :— | :— |
| Primary Symptoms | Severe fatigue, cold intolerance, frequent urination, edema |
| Associated Symptoms | Diabetic nephropathy, neuropathy, retinopathy, poor appetite, weight loss |
| Tongue and Pulse | Pale and swollen tongue with thin white coating, deep and weak pulse |
| Pathogenesis | Prolonged illness consuming both Yin and Yang |
| Etiology | Long-standing diabetes, poor glycemic control, multi-organ damage |
Acupuncture Point Prescription
| Point | Location | Function |
| :— | :— | :— |
| Shenshu (BL23) | Lower back, 1.5 cun lateral to L2 | Back-Shu point of Kidney meridian; tonifies Kidney Qi and Yang |
| Mingmen (GV4) | On the midline, between L2 and L3 spinous processes | Warms Kidney Yang and strengthens the body |
| Taixi (KI3) | Depression between medial malleolus and Achilles tendon | Source point of Kidney meridian; nourishes Kidney Yin |
| Guanyuan (RN4) | 3 cun below the umbilicus | Warms Yang and supplements Qi |
| Yishu (EX-B3) | Lower back, 1.5 cun lateral to T8 | Extra point specifically for diabetes; regulates pancreatic function |

Yin-Yang Deficiency corresponds to advanced type 2 diabetes with significant organ damage, where β-cell failure and multi-organ dysfunction contribute to severe metabolic and systemic decline.
