Pattern Overview
Yin Deficiency with Dryness-Heat is an early pattern in type 2 diabetes, characterized by early-stage β-cell dysfunction, polydipsia, polyuria, dry mouth, and polyphagia. The core pathogenesis is Yin deficiency failing to control Yang, generating dryness-heat, leading to excessive fluid consumption, frequent urination, and hyperglycemia.
Key Symptoms and Differentiation Points
| Differentiation Dimension | Manifestations |
| :— | :— |
| Primary Symptoms | Polydipsia (excessive thirst), polyuria, dry mouth, polyphagia |
| Associated Symptoms | Dry throat, red face, irritability, constipation, weight loss |
| Tongue and Pulse | Red tongue with dry yellow coating, rapid and forceful pulse |
| Pathogenesis | Yin deficiency with dryness-heat, consuming fluids |
| Etiology | Excessive spicy and rich food intake, emotional stress, constitutional Yin deficiency |
Acupuncture Point Prescription
| Point | Location | Function |
| :— | :— | :— |
| Yishu (EX-B3) | Lower back, 1.5 cun lateral to T8 | Extra point specifically for diabetes; regulates pancreatic function |
| Pishu (BL20) | Lower back, 1.5 cun lateral to T11 | Back-Shu point of Spleen meridian; strengthens Spleen Qi |
| Taixi (KI3) | Depression between medial malleolus and Achilles tendon | Source point of Kidney meridian; nourishes Kidney Yin |
| Sanyinjiao (SP6) | 3 cun above the medial malleolus | Nourishes Blood and Yin; regulates the Spleen and Kidney |
| Rangu (KI2) | Medial foot, below the navicular tuberosity | Spring point of Kidney meridian; clears Kidney Heat |
Clinical Recommendation: Filament needle, reducing method for Heat-clearing points, reinforcing method for Yin-nourishing points.
Herbal Formula
Base Formula: Xiao Ke Fang (消渴方) + Yu Nu Jian (玉女煎)
Composition:
Trichosanthes Root (Tian Hua Fen), Coptis (Huang Lian), Rehmannia (Sheng Di), Ophiopogon (Mai Dong), Anemarrhena (Zhi Mu), Gypsum (Shi Gao)
Action: Nourishes Yin, clears Heat, and stops thirst.
Modifications
| Associated Symptoms | Additions/Subtractions |
| :— | :— |
| Severe thirst | Increase Tian Hua Fen, Mai Dong dosage to generate fluids |
| Polyphagia | Add Huang Lian, Huang Qin to clear Stomach Heat |
| Constipation | Add Da Huang to drain heat |
| Weight loss | Add Shan Yao, Huang Qi to supplement Qi and nourish Yin |
Western Mechanism Reference
Early-stage pancreatic β-cell dysfunction causing insufficient insulin secretion, hyperglycemia, and osmotic diuresis leading to polydipsia. Yin Deficiency with Dryness-Heat corresponds to early type 2 diabetes with insulin deficiency and hyperglycemia, where osmotic diuresis and fluid loss contribute to polydipsia and polyuria.
