Pattern Overview
Wind-Cold is a common pattern in TMJ pain, characterized by cold-induced joint capsule and masticatory muscle contraction, jaw pain, stiffness, clicking, limited mouth opening, and aggravation by cold. The core pathogenesis is invasion of Wind-Cold pathogens into the facial meridians, causing Qi-Blood stagnation, muscle spasm, and joint dysfunction.
Key Symptoms and Differentiation Points
| Differentiation Dimension | Manifestations |
| :— | :— |
| Primary Symptoms | Jaw pain, stiffness, clicking, limited mouth opening |
| Associated Symptoms | Facial muscle tightness, weakness in chewing, relief with warmth |
| Tongue and Pulse | Pale red tongue with thin white coating, floating and tense pulse |
| Pathogenesis | Wind-Cold pathogens invading the facial meridians |
| Etiology | Cold exposure, air conditioning drafts, sleeping with the jaw exposed, autumn/winter season |
Acupuncture Point Prescription
| Point | Location | Function |
| :— | :— | :— |
| Fengchi (GB20) | Depression below the occipital bone, between the sternocleidomastoid and trapezius | Disperses Wind-Cold and clears the head |
| Hegu (LI4) | Dorsum of the hand, between the 1st and 2nd metacarpals | Disperses Wind and unblocks collaterals |
| Xiaguan (ST7) | In the depression on the lower border of the zygomatic arch, below the temporal process of the zygomatic bone | Key point for TMJ pain; unblocks the Stomach meridian |
| Jiache (ST6) | In the depression on the lower border of the zygomatic arch, about 1 cun anterior to the angle of the mandible | Local point for TMJ pain; relaxes the masseter muscle |
| Ashi points | Tender spots around the TMJ area | Local pain relief and meridian unblocking |
Clinical Recommendation: Filament needle, even method; moxibustion may be added to warm the meridians and disperse Cold. Combine with heat application and gentle jaw exercises.
Herbal Formula
Base Formula: Juan Bi Tang (蠲痹汤)
Composition:
Notopterygium (Qiang Huo), Angelica pubescens (Du Huo), Cinnamon Twig (Gui Zhi), Gentiana (Qin Jiao), Angelica (Dang Gui), Chuanxiong (Chuan Xiong), Saposhnikovia (Fang Feng), Mulberry Twig (Sang Zhi), Licorice (Gan Cao)
Action: Disperses Wind-Cold, warms the meridians, and relieves pain.
Modifications
| Associated Symptoms | Additions/Subtractions |
| :— | :— |
| Severe cold pain | Add Fu Zi, Xi Xin to warm meridians |
| Limited mouth opening | Add Shen Jin Cao, Tou Gu Cao to relax sinews |
| Facial muscle tightness | Add Bai Shao, Mu Gua to relax muscles |
| Headache | Add Bai Zhi, Man Jing Zi to disperse Wind and stop pain |
| Chronic condition | Add Huang Qi, Dang Shen to supplement Qi and unblock collaterals |
Western Mechanism Reference
Joint capsule and masticatory muscle contraction induced by cold, with elevated EMG activity of masticatory muscles and increased synovial fluid viscosity. Wind-Cold corresponds to TMJ pain with myofascial involvement, where cold exposure increases muscle tension, reduces local blood flow, and increases synovial fluid viscosity.
