Pattern Overview
Qi-Blood Deficiency is a common pattern in restless leg syndrome (RLS), characterized by restless legs at night, fatigue, pale complexion, and poor sleep. The core pathogenesis is insufficient Qi and Blood to nourish the leg tendons, leading to neuronal hyperexcitability, restless sensations, and involuntary leg movements.
Key Symptoms and Differentiation Points
| Differentiation Dimension | Manifestations |
| :— | :— |
| Primary Symptoms | Restless legs, worse at night, relieved by movement, fatigue |
| Associated Symptoms | Pale complexion, shortness of breath, palpitations, poor sleep |
| Tongue and Pulse | Pale tongue with thin white coating, thready and weak pulse |
| Pathogenesis | Qi-Blood deficiency failing to nourish the leg tendons |
| Etiology | Chronic illness, iron deficiency, pregnancy, overwork |
Acupuncture Point Prescription
| Point | Location | Function |
| :— | :— | :— |
| ST36 Zusanli | 3 cun below the patella, 1 finger-width lateral to the tibial crest | Strengthens Spleen and Stomach; tonifies Qi and Blood |
| SP6 Sanyinjiao | 3 cun above the medial malleolus | Nourishes Blood and Yin; regulates the Spleen and Kidney |
| BL20 Pishu | Lower back, 1.5 cun lateral to T11 | Back-Shu point of Spleen meridian; strengthens Spleen Qi |
| KI3 Taixi | Depression between medial malleolus and Achilles tendon | Source point of Kidney meridian; nourishes Kidney Yin |
Clinical Recommendation: Filiform needle, reinforcing method; moxibustion may be applied to Zusanli and Pishu to warm and tonify Qi and Blood.
Herbal Formula

Modifications
| Associated Symptoms | Additions/Subtractions |
| :— | :— |
| Severe restless legs | Add Suan Zao Ren, Ye Jiao Teng to calm the spirit and relax tendons |
| Poor sleep | Add Suan Zao Ren, Yuan Zhi to calm the spirit |
| Cold limbs | Add Gui Zhi, Sheng Jiang to warm the meridians |
| Iron deficiency | Add Dang Gui, Shu Di to nourish Blood |
Western Mechanism Reference
Dopamine dysfunction with reduced neurotransmitter synthesis, impaired iron metabolism, and neuronal hyperexcitability. Qi-Blood Deficiency corresponds to primary RLS with iron deficiency and dopamine dysfunction, where impaired iron metabolism and reduced dopamine synthesis contribute to neuronal hyperexcitability and restless sensations.
