Pattern Overview
Damp-Heat Accumulation is the most common pattern in acute gout, characterized by urate crystal deposition, acute joint inflammation, redness, swelling, burning pain, and fever. The core pathogenesis is accumulation of Damp-Heat in the joints, causing acute inflammatory response and severe pain.
Key Symptoms and Differentiation Points
| Differentiation Dimension | Manifestations |
| :— | :— |
| Primary Symptoms | Redness, swelling, burning pain in the affected joint (most commonly the big toe), fever |
| Associated Symptoms | Thirst, irritability, yellow urine, constipation, limited joint movement |
| Tongue and Pulse | Red tongue with yellow greasy coating, slippery and rapid pulse |
| Pathogenesis | Damp-Heat accumulating in the joints |
| Etiology | High purine diet, alcohol consumption, rich food intake |
Acupuncture Point Prescription
| Point | Location | Function |
| :— | :— | :— |
| Yinlingquan (SP9) | Below the medial condyle of the tibia | Transforms Dampness and drains Heat |
| Sanyinjiao (SP6) | 3 cun above the medial malleolus | Nourishes Blood and Yin; regulates the Spleen and Kidney |
| Quchi (LI11) | Lateral end of the elbow crease | He-Sea point of Large Intestine; clears Heat |
| Taichong (LR3) | Dorsum of the foot, between the 1st and 2nd metatarsals | Source point of Liver meridian; clears Liver Heat |
| Ashi points | Tender spots around the affected joint | Local clearing of Damp-Heat and pain relief |
Clinical Recommendation: Filament needle, reducing method; may combine with pricking and cupping at Ashi points to drain Damp-Heat.
Herbal Formula
Base Formula: Si Miao San (四妙散) + Bai Hu Jia Gui Zhi Tang (白虎加桂枝汤)
Composition:
Atractylodes (Cang Zhu), Phellodendron (Huang Bai), Achyranthes (Niu Xi), Coix Seed (Yi Yi Ren), Gypsum (Shi Gao), Anemarrhena (Zhi Mu), Cinnamon Twig (Gui Zhi)
Action: Clears Damp-Heat, reduces inflammation, and relieves pain.
Modifications
| Associated Symptoms | Additions/Subtractions |
| :— | :— |
| Severe redness and swelling | Add Jin Yin Hua, Lian Qiao to clear Heat and detoxify |
| Burning pain | Add Shi Gao, Zhi Mu to clear Heat and generate fluids |
| Fever | Add Huang Qin, Huang Lian to clear Heat |
| Joint limitation | Add Niu Xi, Mu Gua to strengthen tendons and unblock collaterals |
Western Mechanism Reference
Urate crystal deposition activating the NLRP3 inflammasome, IL-1β release, and acute inflammatory response. Damp-Heat Accumulation corresponds to acute gouty arthritis, where monosodium urate crystal deposition triggers intense inflammatory responses, including NLRP3 inflammasome activation and IL-1β release.
