Pattern Overview
Phlegm-Dampness Obstruction is a common pattern in gout, characterized by high purine diet, hyperuricemia, urate crystal deposition, swelling, heavy sensation, and nodule formation. The core pathogenesis is accumulation of Phlegm-Dampness in the joints, leading to purine metabolism disorder and urate crystal deposition.
Key Symptoms and Differentiation Points
| Differentiation Dimension | Manifestations |
| :— | :— |
| Primary Symptoms | Swelling, heavy sensation, nodule formation, limited joint movement |
| Associated Symptoms | Sticky mouth, poor appetite, bloating, fatigue |
| Tongue and Pulse | Pale and swollen tongue with white greasy coating, slippery pulse |
| Pathogenesis | Phlegm-Dampness accumulation in the joints |
| Etiology | High purine diet, poor diet, alcohol consumption, Spleen deficiency |
Acupuncture Point Prescription
| Point | Location | Function |
| :— | :— | :— |
| Fenglong (ST40) | 8 cun above the lateral malleolus, 2 cun lateral to the tibial crest | Transforms Phlegm and Dampness |
| Yinlingquan (SP9) | Below the medial condyle of the tibia | Transforms Dampness and promotes diuresis |
| Pishu (BL20) | Lower back, 1.5 cun lateral to T11 | Back-Shu point of Spleen meridian; strengthens Spleen Qi |
| Zusanli (ST36) | 3 cun below the patella, 1 finger-width lateral to the tibial crest | Strengthens Spleen and Stomach; tonifies Qi |
| Ashi points | Tender spots around the affected joint | Local clearing of Damp-Heat and pain relief |
Clinical Recommendation: Filament needle, even method; moxibustion may be applied to Pishu and Zusanli to strengthen Spleen and transform Dampness.
Herbal Formula
Base Formula: Er Chen Tang (二陈汤) + Si Miao San (四妙散)
Composition:
Pinellia (Ban Xia), Tangerine Peel (Chen Pi), Poria (Fu Ling), Atractylodes (Cang Zhu), Phellodendron (Huang Bai), Achyranthes (Niu Xi), Coix Seed (Yi Yi Ren), Licorice (Gan Cao)
Action: Transforms Phlegm, drains Dampness, and reduces uric acid.
Modifications
| Associated Symptoms | Additions/Subtractions |
| :— | :— |
| High uric acid | Add Tu Fu Ling, Ku Shen to reduce uric acid |
| Nodule formation | Add Zhe Bei Mu, Shan Ci Gu to soften and resolve masses |
| Swelling | Add Ze Xie, Che Qian Zi to promote diuresis |
| Heavy sensation | Add Huo Xiang, Pei Lan to transform Dampness |
Western Mechanism Reference
High purine diet causing purine metabolism disorder, elevated serum uric acid, and urate crystal deposition in soft tissues. Phlegm-Dampness Obstruction corresponds to gout with hyperuricemia and soft tissue deposition, where purine metabolism disorder and elevated serum uric acid contribute to urate crystal deposition.
